SOCIAL WORKER - Population Health (Temple Physician, Inc. - 40 hours days)

The Social Worker assesses the patient's psychological situation after acute or post acute discharge through telephonic outreach and home visits in order to make recommendations to medical personnel, and provides direct services that ensure the patient's best use of health care services that focus on readmission prevention and supports patient's functional status within their home or post-acute care setting.

Education

Masters Degree : Accredited School of Social Work (Required)

Combination of relevant education and experience may be considered in lieu of degree.

Experience

2 Years experience as a Social Worker in a ambulatory or community setting. (Required)

2 Years experience as a Social Worker in a hospital setting. (Preferred)

License / Certifications

CPR - Cardiopulmonary Resuscitation (Preferred)

SW-LIC - PA Social Worker License (Preferred)
