Experience Requirements :

1 YR of recent Skilled Home Health Experience (at least 1 year within the last 3 years)

Case Load : Adult

Certs REQ : BCLS

License : Must have WA license, or compact license

Charting : HomeCare HomeBase (REQUIRED) / Oasis (Required)

Additional Notes :

Traveler is expected to have their own transportation.

System does a good job grouping clients minimizing drive and windshield time.

Occasional maximum radius of 50+ miles.

Timely communication and documentation required

Documentation tools will be provided, and must be returned.

Interdisciplinary Support :

Patient Diagnoses :

Varying and diverse diagnoses seen but commonly include HF, COPD, DM, wounds, Parkinson's, MS, joint replacements

Special Procedures / Unit Details :

Prefer 1+ year of home health experience. Experience with EPIC remote client is a plus.

Skills needed include wound care, wound vacs, catheter (foley, suprapubic, male and female), venipuncture, and ostomy care. Experience with trachs and pleurex is a plus.

Must be willing and able to travel long distances during the course of a day. We serve 4 counties, Pierce, Mason, Thurston and Lewis and may require LPN to travel to any of these 4 areas. Housing in Olympia or Lacey may be preferred.

Special Equipment :

Laptop and work cell phone will be provided. Work bag with PPE and vitals equipment will be provided.
