job_description.job_card.job_descriptionProvide Therapy Services : Conduct daily individual, group, and / or family counseling sessions with clients. Assessment & Treatment Planning : Perform diagnostic assessments and develop personalized treatment plans following state and accreditation standards. Clinical Coordination : Collaborate with a multidisciplinary team to ensure seamless patient care. Quality Assurance & Compliance : Monitor care to ensure compliance with agency policies and state regulations, reporting concerns to leadership as needed. Documentation & Record-Keeping : Maintain accurate, up-to-date patient records in an Electronic Health Record (EHR) system. Family & Community Engagement : Maintain communication with families, guardians, and referral sources to support client progress. Professional Development : Participate in staff meetings, training sessions, and quality assurance committees to enhance service delivery. Education : Bachelor’s degree in Counseling, Social Work, Psychology, or a related field. Licensure : Active or eligible LSW, LPC, or LCSW license in accordance with the Ohio Counseling and Social Work Board. Experience (Preferred) : Prior experience working with adolescents, teens, and adults with mental health diagnoses. Skills & Traits : Strong clinical skills, non-judgmental attitude, and a commitment to patient-centered care. Compliance : Satisfactory criminal background check and drug screen required.
