Werecurrently partnered with a growing Home Health organization lookingfor PTs in the Monterey Countyarea .

Thisrole is paypervisit which directly allows for higher earningpotential and more autonomy. This roleoffers PPV rates$95105 / visit (plus mileage) with an admin rate of$70 / hr.

KeyResponsibilities :

• Maintain30 points perweek.
• Providesphysical therapy services to patients according to a writtenphysicians plan of care and as defined in the State PhysicalTherapy PracticeAct.
• Preparesand submits clinical and progress summaries based on the attainmentofgoals.
• Preparesand submits a clinical progress summary based on the attainment ofgoals as directed by Organizationpolicy.
• Providesphysical therapy consultation to home families / caregivers whenindicated.
• Providesinservice education programs for nursing organization personnel asneeded.
• Participatesin the peer consultationprocess.
• Supervisesphysical therapy assistants according to organization policy andstateregulations.
• Assessingand evaluating therapeutic / rehabilitative / functional status andparticipation in the development of the total plan of care.Evaluate homeenvironment.
• Assessesfor muscle strength mobility gait ROMpotential forrehab.
• Directingphysical therapytreatment.
• Instructingpatients and families / caregivers in the use and care of therapeuticappliances.
• Determiningpriority needs for physicaltherapy.
• Reportingto physician the patients reaction to treatment or changes incondition.
• Initiatesphysical therapy program and instructs other personnel and / orfamily / caregiver members in certain phases of physical therapy withwhich they may work with a patient as well as instructing them asto the goals of the physical therapy program for the patient byparticipating in caseconferences.
• Maytrain patients in the use of prostheticdevices.
• Identifiespatient and family / caregiver needs for other home health servicesand refers asnecessary.

Requirements

Qualifications :

• Possessesa degree from a baccalaureate or masters program in physicaltherapy approved by an accreditedorganization.
• Licensedto practice as physical therapist by the Physical Therapy Board ofCalifornia.
• Twoyears of appropriate experience as a physical therapist.Community / home health experience ispreferred.
• Demonstratesgood verbal and written communication and organizationskills.
• Possessesand maintains current CPRcertification.
• Licenseddriver with an automobile that is in good working order and insuredin accordance with the organizationsrequirements.

Qualifications : Possesses a degree from a baccalaureate or master'sprogram in physical therapy approved by an accredited organization.

Licensed to practice as physical therapist by the Physical TherapyBoard of California. Two years of appropriate experience as aphysical therapist.

Community / home health experience is preferred.Demonstrates good verbal and written communication and organizationskills.

Possesses and maintains current CPR certification. Licenseddriver with an automobile that is in good working order and insuredin accordance with the organization's requirements.
