We are currently looking for a medical social worker to help ensure the psychological, social, medical and spiritual needs of the residents are met.

Essential Duties and Responsibilities :

• Responsible for the overall management, implementation and coordination of the social services program throughout the continuum of care.
• Assists in the development of care plans and counsels residents and family members.
• Observe and understands the residents’ rights and responsibilities at all times
• Participate in interdisciplinary care team conferences and work with other disciplines in developing and implementing service plans.
• Communicates effectively with other team members, public agencies and other external partners as needed.
• Initiate and update all required assessments per regulatory requirements and Lakewood policy.

Minimum Qualifications :

• Bachelor’s degree in Social Work or other human services field as appropriate and / or be a Licensed Clinical Social Worker in good standing.
• Experience in a Skilled Nursing / Assisted Living community with older adults is preferred.
• An understanding of geriatric care and knowledge of the aging process.
• Strong communication skills oral and written
