Job Description

Join our client’s dedicated Home Health team as a Physical Therapist , where you’ll make a meaningful difference in the lives of patients by providing personalized care in the comfort of their homes. This role combines professional independence with the support of an experienced healthcare team, offering flexibility, growth, and the opportunity to impact patients’ recovery journeys.

Key Responsibilities

  • Conduct comprehensive patient assessments and create individualized physical therapy care plans
  • Implement therapeutic interventions to improve patient mobility, strength, and overall function
  • Educate patients and caregivers on home exercise programs, equipment use, and safety techniques
  • Supervise Physical Therapy Assistants and other clinical support staff as needed
  • Maintain detailed and timely clinical documentation using EMR systems
  • Collaborate with interdisciplinary healthcare teams to ensure coordinated and effective patient care

Schedule & Shift Details

  • Monday–Friday | 8 : 00 AM – 5 : 00 PM
  • One weekend per month
  • Two holidays per year (rotating schedule)
  • No on-call responsibilities
  • Local travel to patients’ homes within the assigned area

    • Qualifications

    Required :

  • Bachelor’s degree or higher in Physical Therapy from an accredited institution
  • Active Physical Therapist (PT) license in Pennsylvania
  • Minimum of 1 year of experience in a healthcare setting
  • Proficiency with Electronic Medical Record (EMR) systems
  • Valid driver’s license and reliable transportation

    • Preferred :

  • Experience in home health or outpatient rehabilitation settings
  • Please include your PA PT license number with your application
