Qualifications :

• Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the State of MN.
• Able to travel within a specific geographic area using reliable transportation with valid drivers license and proof of auto insurance.
• CPR Certification
• 2 years of RN experience.
• 1 year case management experience preferred.
• Home Care, Community health, and a wide range of Medical-Surgical experience preferred.

Territory :

Northeast section of the Twin Cities Metro area (i.e. Cities would include those that are North of Highway 94 and East of Highway 65) within MN.

Schedule :

Full-time position with shifts during the day, from Monday to Friday. Occasional holiday work (two holidays annually) and the possibility of weekend shifts based on staffing requirements.

Responsibilities :

• Visits clients in their homes to assess the quality of care and review clinical documentation.
• Delivers services as prescribed by the physician, following the care plan.
• Conducts initial and follow-up nursing assessments of all in-home clients within established timeframes.
• Collaborates with the patient, their representative (if applicable), and caregiver(s) in crafting and assessing the care plan.
• Engages in continuous interdisciplinary patient assessment.
