Job Title : Registered Dietitian Nutritionist

Location : 100%Remote, EST time zone highly preferred

Shift : Monday-Friday 8 : 00-5 : 00 PM EST

Duration : 12 months, possible extension or conversion

Position Purpose : The Registered Dietician provides clinical education regarding appropriate dietary practices specific to Disease Management conditions. The Dietician assists in structuring clinically indicated dietary plans for the self management and treatment of chronic conditions (anxiety, asthma, COPD, CHF, depression, diabetes).

Job Description :

This team will support our high-risk Diabetes and CHF members. This is included in the QAI for 2025 and 2026. There will be an enhancement of the current Diabetes Chronic Conditions program and a new CHF program. This will support to condition specific client products.

Our team provides Care Management services for members. Our work is telephonic and digital. We conduct outreach to engage members in the program and provide resources / education. Our team offers Core Care Management and several specialty programs like the Chronic Conditions Programs. Members are enrolled on average 90-120 days.

We are setup regionally to support all client markets. We have the support of a non-clinical team, Community Resource Coordinators, and our market level partners.

Typical Day in the Role : The RDN will complete specialty consultations and provide suggestions on appropriate meal plans for our diabetic and CHF members. They will also be required to complete assessments along with care plan development. The caseload will consist of at least 75 active cases. There will be audits on performance and audio calls with the members, with an expectation of achieving 90% or better. Weekly productivity expectations is 100% which will encompass the number of cases brought active, the number of care plan updates completed, and time spent communicating with the members.

Education / Experience :

• Bachelor's Degree in a health related field. required
• 2+ years Disease specific education required

Licenses / Certifications :

RD - Registered Dietician Current, active unrestricted license required Candidate Requirements Education / Certification Required : Bachelor's Degree in a health related field & Registered Dietician Nutritionist Licensure Required : Licensed in the state in which they reside as an RDN
