- Sign-On Bonus : $2500
- Compensation : Salary based on experience with additional bonuses and incentives-Sign-on bonus -Up to $12,000 earned productivity bonus- Relocation assistance provided for FTE!

We would love the opportunity for you to be on our team!DETAILS : - This position will work and spend time in our 2 locations : Hazel Crest and Chicago Heights, about a 10-minute distance between the 2 locations- Provide routine primary care as guided by policies and procedures along with the supervising Physician- Provide Family Planning services as requested according to protocol- Provide prenatal services as requested according to protocol-Provide care to both a pediatric and adult patient panelQUALIFICATIONS : - IL License REQUIRED-Experience as an NP preferred, however, will consider candidates with 3-5 years of RN experience relating to a pediatric patient panel-Comfortable tending to a pediatric patient panel as well as adults
