Job Title :  Social Worker – Adult Medical Day Care

Location :  Lakewood, NJ

Schedule :  Full-Time / Part-Time (Flexible Hours Available)

Position Overview :

We are seeking a compassionate and motivated Social Worker  to join our interdisciplinary care team. The Social Worker plays a key role in supporting the emotional, social, and practical needs of our clients and their families.

Key Responsibilities :

Conduct psychosocial assessments upon admission and as needed

Develop individualized care plans in collaboration with nursing and activities teams

Provide support, counseling, and advocacy to clients and caregivers

Assist with Medicaid recertification and other benefit applications

Maintain accurate and timely documentation in accordance with state regulations

Facilitate discharge planning and referrals to community resources

Lead or assist with group discussions and educational sessions

Participate in care plan meetings and quality assurance reviews

Requirements :

NJ Social Work license (CSW or LSW) required

Strong communication, organization, and documentation skills
