A company is looking for a Lead Population Health Clinical Program Development Strategist.

Key Responsibilities

Leads the design of population health strategy initiatives and programs to enhance care coordination and health outcomes

Monitors care management trends and analytics to support program initiatives and identify high-risk member populations

Collaborates with senior leadership to improve coordination of care and overall health outcomes

Required Qualifications

Bachelor's degree required

5 - 7 years of related experience preferred

Experience in behavioral health program development is ideal

Knowledge of federal and state regulations related to health programs

Experience in managing budgets and strategic planning initiatives
