Your Role and Responsibilities
As a Home Health Psychiatric RN Case Manager, you will :
- Deliver outstanding nursing care to patients with psychiatric and medical needs in their homes.
- Conduct comprehensive psychiatric and medical assessments, providing recommendations to the patient's medical provider.
- Collaborate with patients, families, and interdisciplinary teams to create and implement effective care plans.
- Oversee LPNs, CNAs, and aides to ensure high-quality patient care.
- Educate patients and their families on care plans, medications, and home safety.
- Monitor patient progress and coordinate discharge from services.
