Our Community Behavioral Health Clinic is dedicated to providing comprehensive mental health services to individuals of all ages.

Recognizing the importance of accessibility and support within familiar environments, we have expanded our services to include school-based programs for students in the East Valley.

A New Leaf's Community Behavioral Health Clinic is seeking a dedicated and compassionate Case Manager to join our team. This position offers the flexibility of a hybrid or remote work environment, allowing you to make a positive impact on the lives of individuals in our community from the comfort of your home or on-site.

As a Case Manager, you will provide comprehensive support and services to clients, helping them navigate and access the resources they need to achieve their personal and behavioral health goals.

Hourly Pay : $20 - $21 (depending on years of experience)

Hybrid Schedule : Monday Friday; 8am 5pm

Education & Experience

  • Associate Degree in related field + minimum 2 years of health / human services experience
  • Bachelors degree in related field + minimum 1 year of health / human services experience
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively within a team and with diverse populations.
  • Relevant experience providing family support / living skills face to face with clients, preferred

Responsibilities

  • Coordinating Child and family Teams (CFT) and serving as a facilitator and / or team member, using CFT practice guidelines and ensuring that the Plan is carried out.
  • Developing and maintaining collaborative relationships with clients, their family members and significant support systems.
  • Provide Family Support Services and or Living Skills AHCCCS covered support services to identified clients.
  • Screening, assessing, develop, and implement treatment / service plans under the direction of the Team.
  • Updating and completing staffing notes for multi-disciplinary clinical team reviews.
  • Meet and sustain monthly productivity expectations as determined by clinical leadership.
  • Manage assigned caseload to ensure compliance and coordination of care expectations are met.
  • Collaborating with Child and Family Teams, arranging for appropriate evaluations, provide support to clients / families to ensure appropriate follow-up medication appointment, and referrals and community-based services.
  • Coordinating comprehensive support though in-home and community-based education and interventions to address the families' goals.

Compliance

  • Must be at least 21 years of age
  • Clean 39-month driving record
  • Current auto liability insurance
  • Valid driver’s license with reliable transportation
  • Must pass pre-employment drug test
  • Must be able to obtain Class One Fingerprint Clearance Card upon hire (if there are any uncertainties whether an applicant may have to go through an appeal process;

they are encouraged to complete that process prior to applying, as an invalid card will result in termination of employment)
