Job Description & Requirements RN Case Manager StartDate : 11 / 24 / 2025 Pay Rate : $1800.00 - $2700.00 POSITION SUMMARY – RN Case Manager POSITION DUTIES – Assists inpatients and outpatients and their families in the overall Management of their plan of care across the continuum. MINIMUM REQUIRED QUALIFICATIONS – CO / compact RN license; Graduate of an accredited or state board of nursing approved Registered / Professional Nursing program; Flu vaccine; 3 years RN Case Management experience. LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – Days with weekend rotation, no call. Candidate's choice of 3-12's or 4-10's.
