Facility : Community Rehabilitation Hospital West – 40-bed inpatient acute rehabilitation hospital

Role : LPN (Licensed Practical Nurse)

Specialty : Rehabilitation Nursing

Shift Details :

Schedule : 12-hour shifts (06 : 00–18 : 00 or 18 : 00–06 : 00)

Contract Length : 13 weeks

Weekend Requirement : Yes

Call Requirement : No

Float Requirement : Yes – to nearby facilities

Unit Information :

Beds : 40

Patient Ratio : 1 : 6

Patient Population : Adults (stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, orthopedic injury)

EHR : Epic

Equipment : Standard rehabilitation / acute rehab equipment

Supervisor : Alyse Keys

Requirements :

Certifications : BLS (Basic Life Support)

Experience : Minimum 2 years (IRF or acute rehab preferred)
