About The Position

Job Title : Hospice RN

Hours per Shift : 8 am - 4 pm

Experience : Minimum of 2 years

License : Active nursing license required

Certifications : CPR certification preferred

Must-Have : Strong communication skills

Description :

Utilizing the nursing process, carries out comprehensive assessment of patient / caregiver needs to provide quality care to assigned patients in accordance with State and Federal regulations.

Develops and communicates a realistic plan of care for each patient in accordance with regulations and policies.

Ensures that the patient and caregivers are included in the care plan process.

Actively participates in interdisciplinary team meetings to develop an appropriate individualized plan of care within five days of admission and every fifteen days subsequently.

Serves as Case Manager for assigned patients and actively manages caseloads and visit times in accordance with the plan of care.

Implements medical plan of care, nursing interventions, and procedures as necessary to ensure safe, effective quality care.

Stays up-to-date on pain control, symptom management, and high-tech skills such as IV use and maintenance, wound VAC, and complex wound care.

Takes action to ensure infection control standards are maintained throughout the plan of care.

Delegates duties to nursing staff to ensure that the plan of care is followed and patient needs are met.

Consistently reviews documentation, annotates visit notes, and acts on recommendations in accordance with department policies and procedures.

Evaluates various aspects of the implemented plan of care to meet patient and caregiver needs and ensure overall quality of care delivered.

Maintains up-to-date and accurate documentation of nursing care provided to ensure effective integration of information for use by the healthcare team.

Follows agency protocol with processes, including locking down visits, follow-up notes, and visit completion.

Performs patient and caregiver education to ensure informed patient decisions can be made regarding medication side effects, treatment options, and discharge instructions.

Accurately fills out a variety of required forms in a timely manner and ensures that forms are readily available during visits for timely completion.

Carries out supervisory visits in accordance with regulations and communicates information to the Scheduler in a timely manner.

Assesses patients' needs for aide services, develops and coordinates plans of care with patient / caregiver, and provides ongoing supervision.

Transmits patient incident reports to the Quality Improvement Specialist in a timely manner.

Coordinates safe patient discharges and ensures all appropriate paperwork and patient information is processed properly.

Coordinates a wide variety of patient care with physicians, providers, team members, and vendors to ensure safe and effective care.

Participates in peer review audits and provides and accepts feedback in a professional manner.

Prior to admission, obtains prior authorization for services when needed and engages in ongoing communication with insurance case managers.

Develops and carries out educational presentations within various facilities and with community agencies.

Facilitates transfers of patients and ensures appropriate documentation is completed.

Actively participates in team meetings to maximize effective communication and documentation.

Proactively advocates for patient care issues to ensure quality and type of care is sensitive to each patient and family’s needs.
