• Required Certifications : State licensed
• Required Skills : Collaborative care, Coordinates with internal and external team members, Documents pertinent information to facilitate care coordination, Participates in care conference activities, Case management, Care coordination

Job Details :

• Pay : $2168.05 / week - $2357.65 / week
• Assignment Length : 13-week assignment
• Shift : 5, 8-Hour 08 : 00 - 16 : 30
• At least one year of experience required
