Location : Butte / Dillon Montana (Hybrid In-home / remote)

Position Summary :  The Clinical Manager establishes and implements Granite Peak Home Health Skilled Home Care Processes. The Clinical Manager or qualified designee provides oversight of all patient care services and personnel, including making patient and personnel assignments, coordinating patient care, coordinating referrals, ensuring that patient needs are continually assessed, and ensuring the development, implementation, and updates of the individualized plan of care.

Qualifications :

• Minimum of 2 years of nursing experience in Home Health (preferred).
• Associate’s degree in nursing (required); bachelor’s in nursing (preferred).
• Acceptance of the agency’s philosophy and goals.
• Ability to exercise initiative and independent judgment.
• Management experience and knowledge of Montana Home Care License, CMS Skilled Home Care License, and federal, state, and insurance reimbursement programs.
• Basic computer skills and proficiency with web-based software and electronic health records (Wellsky).
• Must pass a criminal background check by the Department of Human Services and not be on the OIG exclusions list.
• Must have a cellular phone for work-related use.
