Pending license accepted Certifications : Basic Life Support (BCLS) required Must-Have : Willingness to work weekends- Flexibility to float between units based on need- Experience with EPIC charting system- Capability to handle various units such as ED, ICU, Pediatrics, and Specialty Units Job Description : Work as part of a team of case managers coordinating care within an acute care facility.
- Be prepared to float to different units within the facility and potentially to a sister facility nearby.- Handle patient care duties for various units, including emergency, intensive care, and pediatric units.
- Ensure patient placements and discharges align with the appropriate healthcare protocols.- Participate in and complete mandatory orientation and training modules pre-start and annually.
- Be expected to work during major holidays, with commitment required for 3 out of 5 specified holidays. - Maintain updated patient records using the EPIC charting system.
- Communicate effectively with healthcare professionals and patients’ families in coordinating care plans. - Attend required facility meetings and complete all necessary administrative tasks.
Candidates are encouraged to prepare references covering one year of employment within the last three years prior to applying, as clearance requires a full year of references on file.
