About The Position
Weekly Pay Range - $2756 To $3062 Job Title : Social Worker Job Specialty : Social Work Job Duration : 13 weeks Shift : 8-hour Days, Every Other Weekend, First to Float Guaranteed Hours : 40 hours per week Experience : Minimum 2 years of clinical and / or medical social work License : License required for submission Certifications : BLS, MSW / LCSW, 5150 Must-Have : Prior experience in a similar healthcare setting Job Description : Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients.
- Dedicate services to patient and family-centered care, prioritizing self-determination and ethical problem-solving.- Work as part of a multidisciplinary health team to conduct comprehensive assessments and develop integrated treatment plans.
- Collaborate with healthcare team members to implement treatment plans that center on patient care.- Manage crisis intervention, patient / family intervention, high-risk screening, and brief counseling.
- Refer patients for financial or other resource needs and facilitate family / patient meetings with the healthcare team.
- Assist in post-acute placement and engage appropriate agencies or resources for high-risk patients.- Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families.
- Serve on hospital committees and participate in developing and presenting in-services to hospital staff.- Complete required modules before the start of the assignment.
- Rotate work schedule to accommodate holiday requirements, with the expectation to work 2 of the 3 major holidays.- Cover emergency department duties and float as needed based on hospital requirements.
- Participate in floor-specific orientation, especially within the emergency department. Additional Notes : Certification at the county level is required for specific assessments.
- The hospital may exercise the right to cancel up to three times within the contract without prior justification. However, re-engagement during an on-call shift does not constitute a cancellation.
- Experience with the Cerner charting system is necessary.
