Develop treatment plans with involvementfamily / support systems to meet the individual s functional goals. He / She provides direct supervision of support personal as related to patient care.

ORGANIZATIONAL SUMMARY A. Reports to : Inpatient Rehab Manager B. Supervises : none C. Key Working Relationships : Other rehab staff, nursing, physicians, case managers, nursing home and home care staff D.

Primary Customer Groups : Patients, family members, physicians, E. Age / Population of Patients AgesESSENTIAL FUNCTIONS A.

Evaluates plans, directs, supervises and conducts habilitative and rehabilitative programs for patients with physical limitations.

B.Consults and communicates with patient, family, and other multi-disciplinary team members and service providers.C.Records and reports patient s progress towards established goals, and data related to care in the patient s medical record / chart.

D.Instructs patients in exercises, ambulation, and activities of daily living, safe body mechanics, home exercise programs, and return to work precautions.

E.Assists in maintaining the department and equipment in a clean and orderly manner in accordance with department policy regarding infection control and preventive maintenance.

F.Attends and participates in department in-services and meetings.G.Delegates and supervises activities of physical therapy assistants and technicians, to assure efficient use of department resourcesH.

Is actively involved in planning and promoting department growth and program developmentI.Assists in cost containment through efficient use of work time and supplies.

Promotes efficient utilization of resources / supplies. Ensures charges are appropriate based on services provided adhering to productivity guidelinesJ.

Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.

K.Communicates with and performs duties following the philosophy of MercyOne North Iowa Medical Centerand the Sisters of Mercy reflecting the organization s Mission, Values, and Guiding Behaviors.

L.Supports and abides by all medical center, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment.

N.Performs other responsibilities as requested by supervisor, manager or department head.ADDITIONAL FUNCTIONSA.Flexes / floats to other inpatient rehab areas to provide physical therapy services as needs warrant.

POSITION QUALIFICATIONSA.Education : 1.Graduate of Physical Therapy Program.2.Must possess / maintain license in the State of Iowa.

B. year of acute care experience preferred.C.Special Skills and Competencies : 1. Ability to have good relationships with people while dealing with a variety of physical disabilities.

2.Must be able to read, write and / or speak English.3.Completes the following mandatory courses (check all that apply) : XMust provide proof of Basic Life Support (BLS Provider-American Heart Association or Professional Rescuer-American Red Cross) prior to employment or within 30 days of hire and maintains current status.

XMust provide proof of Mandatory Reporting for Child and Dependent Adult Abuse training prior to employment or within 6 months of hire and maintain current status per State of Iowa guidelinesCompletes Advanced Cardiac Life Support (ACLS) American Heart Association within 6 months of hire and maintains current status.

Completes Pediatric Advance Life Support (PALS) American Heart Association within 6 months of hire and maintains current status.

Completes Neonatal Resuscitation Program (NRP) American Heart Association within 6 months of hire and maintains current status.

Completes MANDT Training within 6 months of hire and maintains current status.Completes Trauma Nurse Core Course (TNCC) within 6 months of hire and maintains current status.

Motor Vehicle : MercyOne Vehicle select this for positions required to drive a Mercy owned Vehicle If required to operate MercyOne vehicle, will need to have proof of and maintain valid driver's license on file with Human Resources.
