job_description.job_card.job_descriptionPosition is based in a home health setting across the North Region including Arlington, Everett, Malden, Medford, West Medford, Melrose, and Somerville Floats to multiple locations as assigned within the North Region Day shift schedule from 08 : 00 to 16 : 30 Weekend rotation required once every 6 weeks Care delivered in patient homes according to organizational community health standards Uses Epic EMR for electronic documentation Primary responsibilities include assessment, implementation of care plans, education, and support for patients and families Responsible for patient admissions and ongoing case management with timely electronic documentation Collaboration with interdisciplinary teams to ensure comprehensive patient care Parking is not validated in Boston locations; public transportation and public parking lots are available with daily charges Use of UKG / Kronos for timekeeping Local candidates must permanently reside at least 60 miles from Massachusetts General Hospital in Boston to be eligible Active RN license in Massachusetts required at time of consideration; conditional or pending licenses will not be accepted At least 1 year of certified home care experience Minimum 1 year of case management experience Must hold current AHA certifications Ability to travel to assigned cases within the region Proof of influenza and COVID-19 vaccination required; no exemptions permitted Two professional references required High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
