Pay Range : $38 - $40 / hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

- Building care plans, medication review, assessments care plans.
- 20 calls / day.
Key Requirements and Technology Experience :
- Time Management (work fast-paced environment).
- Works independently.
- Work remotely in telephonic CM.
- Technical skills.
- Associate’s degree.
