Job Duties :

A $3,000 hiring bonus is being offered for this position. Hiring bonuses shall only be for new hires of original employment or reemployment as defined in Human Resources Policy No. 5.0.

  • $750 will be awarded in the first paycheck.
  • $1,000 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of Meets Expectations .
  • $1,250 will be awarded upon completion of the employee’s one (1) year probationary period and a performance evaluation with an overall rating of Meets Expectations .

The City of Virginia Beach Department of Human Services is seeking to hire two Licensed Practical Nurses (LPNs) who have nursing experience working with adults with mental illness, substance use, and or co-occurring disorders to provide primary care screening services to patients in the Adult Outpatient Services Program in the Behavioral Health Division .

Please note that duties do differ for each Licensed Practical Nurse position.

Duties for the first Licensed Practical Nurse(LPN) position :

  • Monitor and screen all clients on anti-psychotic medications for Metabolic Syndrome to include completing extensive documentation in both the paper record and electronic health record (primary care screening) (blood pressure, pulse, weight, BMI, A1C, Glucose Monitoring and Cholesterol Levels).
  • Preform venipuncture, prepare laboratory requisition forms when blood work is ordered and ensures laboratory test results are in the medical record.
  • Track laboratory results but not limited to, vital signs, transcription of physician orders and consultation with Prescriber regarding PCP follow up where applicable.
  • Collaborate with internal and external care providers to provide the highest level of safety, efficient and effective service delivery.
  • Collaborate with prescribers in regards to lab orders and abnormal values.
  • Communicate with outside providers for follow up care for patients; Maintain communication with clients and their families regarding the importance of follow up care with outside prescribers
  • Collaborate with primary care providers for continuity of care and link clients without a primary care provider to a resource to obtain medical care.
  • Address barriers to treatment follow up with clients and families.
  • Provides plan for follow up with clients who no show to PCP appointments and works with clients to keep medical appointments.
  • Maintain clinical records in accordance with regulatory standards, including Department of Human Services, DMAS, and licensure;

obtains client / guardian signatures on required forms. This includes entering progress notes in the EHR for all client interactions including, but not limited to, scheduled appointments, injections administered, calls and correspondence with clients.

Duties for the second Licensed Practical Nurse(LPN) position :

  • Provides nursing services to a caseload comprised of adults with mental health, substance use or co-occurring disorders who are receiving medication management services in an outpatient setting to include ongoing assessments, crisis intervention, medication education and management.
  • Provides preparation for physician visits, administration of oral and IM medications, vital signs, transcription of physician orders, ordering and packaging medications.
  • Provides education / counseling regarding medications and relevant nursing domains.
  • Provides assistance to clients as assigned and support to prescribers as directed in the process of procuring medications including but not limited to processing requisitions for medications, calling in prescriptions to pharmacies, coordinating with patient assistance program staff, and staging prescriptions for prescriber final approval.
  • Collaborates with primary care providers for continuity of care and links clients without a primary care provider to a resource to obtain medical care.
  • Completes extensive documentation in both the paper record and electronic health record (EHR) including medications prepared for client, injections administered to client, primary care screening (blood pressure, pulse, weight, BMI, waist circumference, etc.)
  • Provides coverage for Behavioral Health nursing vacancies as needed.
  • Designated as clinic site Medication Room Nurse, responsible for following the guidelines necessary to maintain the Virginia State Board of Pharmacy Controlled Substance Registration and ensuring compliance with Medication Room policies.
  • Collects and documents urine drug screens and breathalyzers as directed by the prescriber, ensuring that all filing of clinical information and submission of billing information is done accurately and within the required timeframe.
  • Prepares laboratory requisition forms, enters lab tests ordered by the prescriber in the electronic health record or the laboratory web site, and ensures laboratory test results are in the medical record.
  • Maintains clinical records in accordance with regulatory standards, including Department of Human Services, DMAS, and licensure.

Minimum Requirements :

MINIMUM : Possession of a current and valid license issued by the State of Virginia or as a part of the Compact Designation (Multi-State Privileges) as a Licensed Practical Nurse and one or more years of experience as a Licensed Practical Nurse to be specified by the hiring department.

Current CPR certification. Must have a current and valid driver’s license. SPECIAL REQUIREMENTS : All employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and / or City-wide emergencies.

Preferences :

At least two (2) years of nursing experience working with adults in an outpatient or inpatient behavioral health setting.

At least one (1) year of experience administering medications in a psychiatric setting, including administering long acting injectables.

At least one (1) year of experience documenting nursing services in an electronic health record.

At least one (1) year of experience performing venipuncture.

Experience providing patient education on various health conditions to individuals in a clear and understandable manner.

Experience supporting individuals navigate the healthcare system to link with medical providers.

Experience utilizing spreadsheet software, such as Microsoft Excel, to effectively manage and organize data.
