One of our clients is seeking a Multi-Site Servicing RN Case Manager at an IDD Facility located in the Northeast Philadelphia area.

CLIENT'S AVAILABLE HOURS

8 : 30am until 5pm, Monday through Friday

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY

• In this role, you will have the opportunity to provide case management support to individuals in group homes, while working closely with the patient care team via zoom and in-person meetings.
• Provide clinical assessment, and medication administration to patients.
• Go to group homes to deliver medication, update patient treatment logs, and medical charts.
• Partner with healthcare professionals to coordinate patient care.
• Perform other duties as assigned.

CLIENT'S REQUIRED SKILLS & EXPERIENCE

• Graduate from an accredited Nursing Program.
• Current RN license in the state of PA.
• Possession of a current and valid PA driver license.
• Previous experience and / or interest working with individuals with physical and developmental disabilities.
• Ability to perform professional nursing duties, including but not limited to assessments, treatment, case management, etc.
• Experience with outlook, MS Office, MS Word is REQUIRED!
