Job Title : Social Worker Job Specialty : Case Management Job Duration : 13 Weeks Shift : Sunday through Wednesday, 7 : 00 AM - 5 : 30 PM (10-hour shifts) Guaranteed Hours : Not specified Experience : Experience in an emergency department is required License : Must have applicable social work license for the job location Certifications : 5150 Certificate required Must-Have : Experience in a fast-paced environment Job Description : Provide case management services and support for clients- Assist clients in navigating available resources and services- Collaborate with healthcare professionals and community organizations- Document and maintain detailed records of all client interactions and progress- Advocate for clients in accessing necessary social services and interventions- Utilize critical thinking and problem-solving skills in challenging situations- Maintain confidentiality and ethical standards in all interactions and documentationThis position offers an opportunity to work collaboratively within a dynamic team to support individuals with complex needs.
