The Clinical Social Worker - Maternal and Child Health provides professional services to clients and their families eligible for the Maternal and Child Health Services in order to meet identified psychosocial, emotional, financial, and environmental needs.
The social worker conducts psychosocial assessments, offers counseling and crisis intervention, provides financial resource information, and makes referrals to community agencies for clients and their caregivers / families in various settings.
This role involves collaborating with interdisciplinary teams, facilitating care transitions, and implementing interventions at the individual and systemic levels.
Responsibilities :
- Engage clients and families in case planning decisions
- Provide social work intervention to eligible clients and their families
- Facilitate care transitions and referrals to social services and specialists
- Keep management informed of problematic cases, especially those involving legal or risk management issues
- Assist clients in obtaining financial resources and government entitlements
- Develop and maintain tracking system of social services referrals / outcomes
- Provide information and counseling for advance directives and health care power of attorney
- Respond to referrals from healthcare team members and collaborate with nurse home visitors and mobile health clinic staff
- Communicate details to enhance service / care across service lines and among disciplines
- Document all client interactions, significant observations, interventions, and actions taken in the client's medical record
- Facilitate education / training modules to assist RN staff managing basic social work needs
- Participate in community coalitions and promote public awareness of Maternal and Child Health Services
- Develop and maintain community relationships to support client referrals
- Assess clients' and families' needs and assist in establishing goals and outcomes through therapeutic relationships
- Consult and collaborate with other professionals involved in providing services to women and families
