Case Manager

REPORTS TO : THV Manager of Supportive Services

HOURS : Full-time, 40 hours per week, includes some on call evenings and weekends

PAY RANGE : $28.35 - 33.60 per hour, DOE

BENEFITS : Medical, dental, vision, retirement, life insurance, long-term disability insurance and sick leave are available to employees.

Employees accrue 116 hours of vacation during the first year and receive 11 paid holidays. (Prorated for part-time employees).

POSITION TYPE : Non-exempt position. This is unionized position, represented by Local 8 of the Office of Professional Employees International Union (OPEIU)

The Case Manager is responsible for assisting homeless individuals to stabilize and secure permanent housing. Short-term service planning, maintaining relationships with community agencies and service providers, brief intervention and crisis response, providing informal counseling to clients as needed, information and referral assistance, and coordination of community-building activities.

Responsibilities :

Information and Referral / Case Management

  • Provide information and referrals to clients, including through distribution of written material.
  • Maintain updated information tools for tenant use in accessing community resources.
  • Formally and informally, assess clients as to current level of functioning and social service needs.
  • Make referrals or initiate services to address mental health, substance abuse, and disability issues wherever possible.
  • Coordinate with local agencies to provide services that meet the needs of LIHI clients.
  • Work collaboratively with Shelter Operations and Supportive Services staff to increase stability.
  • Build effective relationships with clients.

Community Building

  • Coordinate with community organizations, public agencies, and volunteers to provide on-and off-site activities for clients.
  • Initiate and encourage client involvement in community-building activities, such as community meetings, meals, and support groups.
  • Produce needed written materials, including community newsletters.
  • Contribute to the development of a community that values cultural diversity.

Administrative

  • Participate in staff meetings and planning sessions.
  • Report regularly to Supportive Services Supervisor on programmatic issues.
  • Maintain records of activities and contacts with clients / agencies and case management records.
  • Complete other records and reports appropriate to the position as directed.
  • Assist staff with administrative and program work related to social service projects at LIHI.
  • Assist with preparation of spreadsheets, grants, funding applications, correspondence and reports as relate to the shelter programs.

Other

  • Participate in public policy advocacy efforts including recruiting clients to attend public meetings and testify at hearings.
  • Participate in fundraising for Supportive Services by providing information and feedback for grant applications; and helping establish community contacts.
  • Perform other related duties as required.

Qualifications :

  • Bachelor Degree in Social Work or related field, or Associate degree with 3 years of experience.
  • At least two years' experience working with a diverse population.
  • CPR / First Aid desired
  • Background in case management, resident advocacy, or related career is required
  • Knowledge of housing resources and Coordinated Entry.
  • Knowledge of referral resources
  • Experience working with confidential information
  • Excellent written and communication skills
  • Access to transportation
  • Flexibility to work evenings or weekends as needed
  • Must pass criminal background checks and drug screening test.
  • Working knowledge of Veterans Administrative resources, HUD-VASH guidelines and Grant Per Diem projects highly desired.
