Annex Healthcare, a leading home healthcare agency in Southern California, is seeking dedicated and passionate recent LVN graduate s to join our team.
As an Entry-Level Home Health LVN, you will provide in-home care to pediatric or adult patients with complex needs, including G-Tubes, Trachs, and ventilators.
This role offers flexible shifts (8, 10, 12 hours), allowing you to create a schedule that suits your needs and priorities.
Locations : La Crescenta, Granada Hills, Agoura Hills, Canoga Park, Sun Valley, Woodland Hills, A Glendale, Los Angeles, Santa Clarita, San Fernando valley and Lancaster.

Responsibilities :
- Provide in-home care to patients with illnesses, injuries, and chronic diseases under the supervision of an RN Case Manager.
- Administer medication, manage chronic illnesses, and assess changes in patients' medical status.
- Assist patients with daily living activities and maintain their activities of daily living.
- Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor.
- Collaborate with physicians, other agency staff, and contract services to coordinate and implement the Plan of Care.
- Complete all required documentation in accordance with agency standards and regulatory requirements.

Requirements :
- Current CA state license to practice as a Licensed Vocational Nurse.
- Recent graduate from an accredited LVN program.
- Current BLS / CPR Certification.
- Strong desire to provide compassionate care to patients in the comfort of their own homes.
- Ability to work in various conditions and travel to patients' residences.
- Physical ability to stoop, bend, lift, and transfer patients.
