We are a comprehensive team comprised ofRNs Physical Therapists LPNs LNAs SLPs Social Workers andadministrative support all collaborating together to supportpatients to remain in living in the setting of their choice theirhome.
The Community Health Nurse LPN will beresponsible for the delivery of basic nursing care to patients in ahome care settings under minimal clinical supervision as directedby a Registered Nurse and in accordance with an established nursingplan of care.
The general territory for thisposition is Burlington Winooski and South Burlington. At timestravel may be required outside of assigned territory but a generalservice area is assigned and part of typical travel.
All mileage isreimbursed at the federal gas rate(.65c / mi)!
Qualifications :
- AccreditedSchool of Nursing LPN Program
- Two or more yearsof directly related home / public health and / or hospiceexperience
- Current LPN licensure or compactlicensure recognized by the State of Vermontrequired
- Previous home caredesired.
EssentialPhysicalRequirements :
1.Must walk sit stand bend lift and move intermittently duringworking hours
2. Must have theability to push pull and lift from the floor up to a total of 50pounds and be able to move with the weight as to organizationsstandards
3. Climbingbe able tonavigate stairs of varying heights anddegrees
