Case Manager

Pay Rate : $21 / hr.

Location : Houston, Texas 77006

Hours : Monday Friday 8 : 00am 5 : 00pm

A non-profit organization is seeking a Case Manager to join their team. With your strong organization and communication skills, you will profoundly impact the organization by conducting intakes, providing orientations, providing services, and maintaining documentation.

Responsibilities :

• Assess client eligibility, complete intake and orientation, and create individualized service plans.
• Assess whether a client is eligible to receive social services benefits.
• Ensure that services are provided in accordance with LEP policy.
• Advocate for clients with social service agencies.
• Maintain client’s file and case notes.
• Perform duties as assigned such as assisting with outreach and participating in : staff retreats, United Way tours, agency fairs, etc.

List Qualification or Requirements :

• High School Diploma or GED required.
• One or more years of experience in an administrative setting or within refugee populations.
• Must have a valid Texas driver’s license.
• Intermediate proficiency in Microsoft Office Suite.
