DEPARTMENT SUMMARY : The Los Angeles LGBT Center’s Department of Health Services is one of the nation’s largest and most experienced providers of LGBTQ-specialized primary care and related services, Our mission to help LGBTQ people lead healthier, happier lives is supported by our comprehensive array of client-centered services including primary care, mental health, psychiatry, sexual health, pharmacy, case management, nutrition, clinical research, housing services and a food pantry.

The Center is one of a handful of Federally Qualified Health Centers in the nation specializing in serving LGBTQ people.

JOB SUMMARY : Under the supervision of a collaborating physician, this nurse practitioner (NP) will provide comprehensive, trauma-informed, and culturally competent medical care to LGBT individuals throughout the adult lifespan.

The NP’s panel of patients will reflect the diversity of LGBT community and include individuals of all genders and expressions and with varying levels of medical and psychosocial complexity, including those living with HIV.

The NP will also be expected to provide episodic care for non-panel patients as needed.

ESSENTIAL FUNCTIONS :

  • Under the clinical supervision of a collaborating physician, provides comprehensive medical care to a panel of patients including primary care, chronic disease management, sexual and reproductive health care, and gender affirming services.
  • Provides ongoing management to adults living with HIV including : diagnosing acute HIV infection; initiation and modification of appropriate antiretroviral therapy;

prevention and treatment of acute opportunistic infections; monitoring clinical and immunological status; routine preventative health screenings unique to HIV patients.

  • Conducts clinical encounters with new, return, and cross-coverage patients including taking an HPI (history of present illness);
  • completing physical exams; documenting medical, social, family history; ordering diagnostic tests and interpreting results;

developing differential diagnoses; and establishing and documenting plans of care.

Uses clinical judgment and consults with collaborating physician when necessary to assess the need for specialist assessment and places referrals accordingly.

Conducts telephone consultations with specialists as needed. Reviews specialists’ notes, incorporating findings and recommendations into the overall management of the patient.

  • Demonstrates chart maintenance by keeping patient problem lists up-to-date and reconciling medications.
  • Follows practice documentation standards regarding clinic notes and clinically relevant lab results and imaging reports.
  • Completes and signs-off on clinical notes, lab results, refill requests, imaging reports, and consults within a timely manner.
  • Responds to messages from patients and care team members in a timely manner.
  • Provides evidence-based care that is in accordance with national standards and internal practice guidelines.
  • Seeks consultation from staff physicians as needed, especially when a patient’s problems are especially complex or if the patient’s condition is not responding as expected.
  • Participates in peer review exercises and completes mandatory clinical trainings.
  • Liaisons with other members of the clinical care team including nursing, social work and mental health to optimize patient health outcomes.
  • Provides cross- coverage for the other medical providers in cases of vacations, conferences, and illness, and overflow.

This includes review of other providers’ lab results, portal messages and medication refill requests.

  • Attends staff and other meetings as assigned.
  • Other duties as assigned.
  • Maintains sufficient Continuing Education Units (CEU) as required by the California Board of Nursing for license renewals and as may be required to maintain any certifications necessary to demonstrate proficiency in a specialty area relevant to NPs clinical practice.

JOB QUALIFICATIONS AND EXPERIENCE :

  • Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people.
  • A passion for the Center’s work and its mission to make the world a better place for LGBTQ people.
  • Excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills.
  • Completion of nurse practitioner training from an accredited institution and a license to practice in the state of California
  • Minimum 2 clinical experience.
  • Current DEA license and BLS certification.
  • AAHIVM certification or ability to become AAHIVM certified within 12 months of employment.
  • Commitment to providing care that is trauma-informed, sex and body positive, and harm reductive.
  • Strong sexual and reproductive health experience including expertise in STD diagnosis and treatment, PEP / PrEP, gender affirming hormone therapy and contraception management, preferred.
  • Ability to conduct medical visits in Spanish, preferred.
  • Understanding of the intersectional issues impacting LGBTQ people including the ways that racism, sexism, transphobia and income inequality impacts health access and outcomes.
