The Licensed Nurse is responsible for supervising the personal care staff; coordinating all personal care services provided to residents by personal care staff;

promoting the health and well-being of each resident; reporting observed changes in resident condition or health to the Executive Director and resident's responsible party.

Essential Functions

  • Possess an active licensed nurse certification (Registered Nurse) within the state where the home building resides
  • Leadership through planning, directing, communicating, and coaching. This position requires strong written verbal communication skills, strong personnel skills, and positive community relations skills to be successful
  • Excellent writing, verbal grammatical, and punctuation skills
  • Must be self-motivated and have the ability to work independently and make decisions.

Responsibilities : Duties

Duties

Clinical Oversight :

The Registered Nurse performs a thorough health assessment on all new residents upon admission to the community. The RN will review admission orders and resident profiles to ensure the accuracy of information (including but not limited to diet, DNR status, allergies, diagnosis, services, etc.)

The Registered Nurse is responsible for monitoring the health status of all current residents through assessments and care conferences scheduled quaterly or more frequently as required (30 days reviews, 90 day reviews, changes in condition).

Resident assessments will be completed by the RN (consulting with ED, HSD, and RCC).

This RN will ensure care plans will be entered into Eldermark and printed by due dates and as needed for changes in condition and the RN will monitor compliance with those care plans by all staff.

This monitoring includes actions required to improve their health status and reading of all alert charting from daily reports.

This monitoring is to include but not limited to : medication side effects, activities, meals, complaints, cognition and behaviors, ADLs and special requests.

Using this information, the RN will coordinate and attend resident care conferences and service plan reviews, as requested by the Executive Director, HSD, or RCC, with a focus on assuring that the service plans are developed to meet the resident's needs.

The Registered Nurse will review all doctor orders (RN check) for psychotropic medications the RN will conduct a focused assessment and enter a nursing note per state regulations regarding psychotropic medications.

The Registered Nurse will react immediately to suspected resident abuse including a thorough investigation and notification of appropriate parties as needed.

The Registered Nurse will perform skin assessments and enter weekly skin notes for residents with skin issues. The RN will ensure the skin protocol is followed including checking for or entering skin treatments on the MAR.

Coordination Of Care- The Registered Nurse will communicate with PCPS, Home Health agencies (PT, Wound care, etc.), and hospice in accordance with residents' needs.

Monthly Monitors - The Registered Nurse will review monthly monitors for all residents, coordinate with PCPs, and enter chart notes accordingly (vitals, weights, pain, skin, falls, hospitalizations, coordination of care, diabetes, blood thinners, supportive devices with restraining qualities, etc.)

Delegations- The Registered Nurse will ensure delegations are in place for residents needing subcutaneous injections (such as insulin-dependent residents) The Registered Nurse will ensure delegations are done in accordance with Oregon Regulations- Board of nursing (chapter 851, Division 47).

The Registered Nurse will perform other nursing tasks / notes / assessments, etc. to ensure the community is in compliance with Oregon state regulations.

Supervisory / Administrative Duties :

The Registered Nurse will attend weekly management meetings, med tech meetings, and other meetings as needed. The Registered Nurse working with the Health Services Director and Resident Care Coordinator will participate in facility audits (including the Medroom) as required of current operational systems and monitor staff compliance with policy and procedure.

The Registered Nurse may perform all necessary verbal and written disciplinary actions required of supervised staff (med techs) and ensure documentation and process meets Bear Creek policies and procedures.

The Registered Nurse, health services director, and resident care coordinator will review all incident reports, conduct investigations, and report / plan corrective action as required.

Continuous Quality Improvement :

The Registered Nurse working with the health services director, resident care coordinator, and Executive Director will assist in ensuring there is a Continuous Quality Improvement (CQI) activity in the building.

Also, the RN will assist in will assist in identifying and setting priorities in resolving service-related issues, and those CQI activities within the community to meet Bear Creek's Goal of quarterly meetings, and document plans to improve quality.

Compliance With Rules And Regulations :

The Registered Nurse will support the HSD and RCC in maintaining compliance with all local, state, federal, and licensing rules and regulations associated with the licensure of the building as they pertain to clinical and risk management activities.

This includes HIPPA, OSHA, and state survey issues, etc. that apply to specific locations of the property

Safety Compliance :

The Licensed Nurse is responsible for ensuring that employees work in a safe manner and that unsafe actions are to be managed.

Compliance with safety committee standards is required. Should workplace injuries occur, the Executive Director would be responsible for following all procedures for treatment, investigation, return to work, and management of the injured worker, communication with care provider and Bear Creek.

Team Effectiveness :

The Licensed Nurse is responsible for fostering teamwork with coworkers, showing initiative in overcoming obstacles, take actions to assist others beyond job responsibilities, and has a positive impact on co-workers, residents and visitors.

Qualifications :

Education & Licensing

  • Registered Nurse
  • State-specific accreditation or license required
  • Associate's or Bachelors's degree strongly preferred in business or a field related to the position

Experience

  • Must have proven manager experience and ability to manage staff
  • Minimum three years experience in assisted living, Alzheimer or memory loss care
  • Experience in residential care management, Clinical Director, or Risk management
  • Experience with Microsoft Word and Microsoft Excel

Working Conditions

Professional office environment. Uses PC, telephones, word processing, and other standard office equipment. Verbally able to articulate in person and over the phone.

Attending business-related functions.

The Licensed Nurse encounters stressful conditions dealing with residents and employees. Must be able to work with HSD, RCC, and community management staff, in organizing and implementing programs and training in clinical and risk management areas.

Excellent verbal and written skills, interpersonal and mediation skills, ability to evaluate workers' performance, and strong working knowledge of employment laws and company regulations.

Physical Requirements :

Sitting, walking bending, data entry, reading, written and verbal communication, and lifting up to 50 pounds
