Supervises the staff and day to day operations of providing care to chronically ill patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes.
Job Responsibility
- Tracks and coordinates with primary care physicians for forms and orders; tracks registered nurse required documentation as noted in nurse database and follows-up on receipt of outstanding documents.
- Engages nursing management for any potential gaps in nursing documentation.
- Assists with field assessment in Client’s home to assess Client’s health status and needs Completes and submit assessment paperwork in a timely and organized manner.
- Assists with carrying out supervisory visits to assess the competency and performance of Home Health Aides or Licensed Practical Nurses.
- Assists in the prevention and detection of issues related to regulatory non-compliance.
- Assist with on-boarding and orienting new field staff.
- Assist in handling auditors, insurance partners, and other government agencies Reports compliance findings quarterly.

Job Qualification
- Bachelor of Science in Nursing required.
- Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
- 4-6 years of relevant experience and 0-2 years of leadership / management experience, required.
