Description
As a Clinical Pharmacist for our Specialty division you will have the ability to provide exceptional customer service to patients and healthcare professionals while providing leadership and guidance to supporting staff.
Drive key clinical and sales initiatives, build partnerships and manufacturers and / or additional clients, and help develop initiatives to grow and retain our specialty patient base.
Duties and Responsibilities
Patient Assessment : Conducting thorough assessments of patients to determine their suitability for home infusion therapy.
This may involve reviewing medical records, evaluating lab results, and assessing the patient's overall health status
In collaboration with the nursing department, ensure employing a home health nurse prior to the start of care if appropriate
Work in close conjunction with healthcare providers, including physicians, nurses, pharmacists, and discharge planners, to coordinate the transition of patients from hospital or clinic-based care to home infusion therapy
In collaboration with the billing department, verify patients' insurance coverage and ensure that the prescribed medication is covered, obtain necessary approvals or explore alternative options if coverage is not available.
Review the prescriptions for accuracy, completeness, and appropriateness, ensuring that the prescribed medication is safe and effective for the patient
Communicate with healthcare professionals to clarify prescription details or obtain additional information.
Draft formulas based on clinical experience and practice guidelines
Ensure that the prescription is properly authorized and meets legal and regulatory requirements
Maintain and update patient profiles in the pharmacy's system. This includes recording relevant medical history, allergies, and other pertinent information to ensure safe and appropriate medication use
Overseeing inventory management, including ordering and maintaining an adequate supply of infusion medications and related supplies
Ensure that pharmaceuticals are accurately compounded and dispensed in accordance with company protocols and physician orders
Determine appropriate selection of supplies
Document medication errors for Quality Assurance review.
Participate in surveys conducted by authorized inspection agencies
Report product incidents as required to the appropriate management official
Manage pharmaceutical recalls in a safe and compliant manner
Participate in the company’s Performance Improvement program.
Pursue continuing education appropriate to job responsibilities and license requirements
Perform other duties as assigned by the supervisor
Assume on-call responsibilities when required
Required Qualifications
Clinical Specialty Pharmacy and / or Hospital Clinical Pharmacy Experience Required.
Ability to prepare, organize, and delegate activities of subordinates.
Exceptional organizational skills that ensure accurate maintenance of records.
Ability to comprehend alternatives and make efficient decisions.
Must possess current knowledge of all applicable state and federal pharmacy laws, rules and regulations.
Microsoft Office proficiency.
Excellent Customer Service and communication skills
General mathematics skills
Drug nomenclature and / or medical terminology knowledge
Specialty pharmacy experience
Infusion supply knowledge
Able to simultaneously manage and prioritize multiple complex projects
Solid attention to detail
Education and Experience Requirements
