Job Title : Physical Therapy Assistant Job Specialty : Rehabilitation Therapy Job Duration : 13 Weeks Shift : Monday-Friday, Days Guaranteed Hours : 32 Hours Experience : Open to New Graduates, Skilled Nursing Facility (SNF) Experience Preferred License : State License Required Certifications : N / A Must-Have : - Completed prescreen attached upon submitting the application- Resume- License verification- Covid test 72 hours prior to start date with results issued 48 hours prior to start date Job Description : Ensure prescreen is filled out and attached when submitting the candidate- Check that resume and license verification are provided- Conduct state license verification- Gather references (3)- Complete county criminal background check (7 years for all counties lived and worked in)- Federal criminal background check (7 years or more)- Multi-state criminal search (7 years or more)- Nationwide sex offender search (7 years or more)- Conduct HHS / OIG search- GSA search- Social Security Number trace- Verify health requirements : 1 Step PPD or Quantiferon, or proof of positive PPD and chest X-ray- Conduct SAM search- Complete Virginia State Police background check- Fill out mandated reporter form- Sworn statement required Position Urgency : Normal Bilingual : Not Required Holiday Coverage : Not Required
