Health Link Home Health and Hospice, is a renowned provider of comprehensive and industry leading home health services in the greater San Francisco Bay Area.
For the past 2 decades, we have been and continue to to deliver high-quality care for our patients. We are currently looking for a compassionate, organized, and team-oriented individual to fill the role of Full-Time Occupational Therapy (OT) for the city of Walnut Creek and surrounding areas.
You will work closely with our multi disciplinary team to ensure coordinated patient care. Our clinical and office staff in Walnut Creek work together harmoniously to deliver outstanding care.

What You'll Do :
Will provide services within the scope of occupational therapy, in accordance with the plan of care, and in coordination with the Interdisciplinary Team
Will follow plan of care; provides treatment; evaluate patient's reaction to treatment and report any changes in his / her condition to the Case Manager and / or Director of Nursing
Online Charting in Kinnser in a timely manner
Be responsible for following all company policies and procedures
Pay Rate :
$60.00 - $72.00 per hour (depending on experience)
$10,000 Sign-On Bonus

Requirements :
Current OT License to practice in CA - Required
- Home Health : 1 year - Preferred
- Current CA Driver's License
Able to travel within a 15 to 25-mile radius on a regular basis
Coverage area Walnut Creek and surrounding areas including Tri-Valley
