Overview
The Manager, Post-Acute Care (PAC) Network is a key leadership position executing on Novant Health’s care coordination strategies related to post-acute care.
Reporting to the VP, Care Coordination and Post Acute, the Manager PAC Network will be responsible for developing and managing the preferred Post-Acute Care Network for Novant Health across all core markets including relationships with skilled nursing facilities, home health agencies, and any other key post-acute stakeholders.
The Manager will set direction, optimize a metric-driven evaluation process, execute formal partnership agreements and build relationships with internal and external partners across the care continuum.
This position will be responsible for developing system-wide strategic and operational plans for post-acute care coordination in partnership with physician leaders, case management leaders and other administrative leadership across the system to include such innovative concepts such as : PAC network tiering, leveraging data in payor relationships, benchmarking discharges to post-acute, increasing PAC network utilization, readmission reduction initiatives and improving transitions across the care continuum.
The Manager will have leadership responsibility for all teams supporting the Post-Acute Care Network including PAC Network care coordinators and provider groups serving as medical directors in partner facilities.
In addition, this position will be responsible for developing business plans, prioritizing initiatives and piloting and implementing various projects designed to advance the care coordination for NH and partner to support value-based care delivery capabilities of Novant Health related to post-acute care.
This position is also responsible for coordinating with payor initiative teams to ensure implementation is consistent with the strategic direction and goals of the organization.
