Summary :
Presbyterian is hiring an RN Care Manager for the Las Estancias Medicine Clinic. The Care Manager pro-actively manages the care for patients along a continuum by facilitating collaborative care between patient, family / support system, physicians, and other ancillary departments / members of the primary care team
Qualifications :
Credentials : Registered Nurse - State of New Mexico or Compact State Nursing License required.
1-3 years ambulatory experience
Education : associate degree in nursing
BLS required at time of hire.
