Description and Responsibilities
The Intensive Community Wrap Around Supports program is designed in partnership with the Department of Developmental Services and the Department of Mental Health to create intensive, in-home and community wrap-around services to support adults between ages 18 and 30 with Autism Spectrum Disorder and Serious Mental Illness, or other significant mental health challenges.
The goal of this program is to maintain individual stability and effective functionality for people served in their homes and communities by providing clinical services and coaching supports to identify and respond to barriers of obtaining stability and support individual progress while building skills and capacity for everyone.
The Registered Nurse is responsible for conducting psychiatric assessments, assessing physical health needs, making appropriate referrals to community physicians, and providing management and administration of medication.
They collaborate with the team, individuals enrolled in services, and their collaterals to develop and revise comprehensive assessments and individualized treatment plans.
Other Key Responsibilities :
- Manage pharmaceuticals and medical supplies.
- Participate in, conduct part of, or take the lead in the development, writing, implementation, evaluation, and revision of comprehensive assessments and individualized treatment plans in collaboration with the muti-disciplinary team and individuals served.
- Provide clinical and non-clinical support in the form of psychoeducation to individuals and families, educational and vocational supportive counseling, assisting with ADLs, supporting individuals to maintain safe and stable housing, teaching life skills, and symptom management.
- Participate in on-call rotation.
Qualifications
- Registered Nurse licensed in Massachusetts, required.
- Minimum of 3 years experience working with individuals with psychiatric issues, ASD, and other developmental challenges, required.
- Valid Drivers License, acceptable driving record, and reliable transportation, required.
