As an entry-level Caregiver - Home Health Aide, you work directly in a client's home to provide robust, around-the-clock care.
You're a helper, a caregiver, and a companion, doing whatever you can to keep your client happy, safe, and healthy. Whether you're preparing meals, going shopping, or assisting with various daily living activities and self-care tasks, you maintain a kind and positive attitude.
RESPONSIBILITIES :
- Perform home visits for patients as delegated by the care managing RN
- Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition
- Supports the patient and family as appropriate to the patient’s condition
QUALIFICATIONS :
- Current certification as a HHA in Connecticut
- Valid driver’s license
- Reliable transportation and willing to travel in New London County
- Strong communication and interpersonal skills
- Home health experience preferred
