Compensation : $70-000 - $75,000 Per Year Salary + Bonus Opportunities

Position Summary :

• Licensed Independent Clinical Social Worker (LICSW), you will conduct comprehensive psychosocial assessments of geriatric residents in nursing homes and assisted living facilities to identify their unique needs, preferences, and psychosocial challenges
• Collaborate closely with residents, their families, and interdisciplinary care teams to develop individualized care plans that address social, emotional, and mental health concerns effectively
• The Licensed Independent Social worker will provide evidence-based therapeutic interventions, counseling, and support services to residents facing various mental health issues, including depression, anxiety, grief, and adjustment disorders
• Advocate for the rights and dignity of geriatric residents, ensuring access to appropriate community resources, social services, and supportive care options
• Foster a supportive and inclusive environment within long-term care facilities, promoting resident engagement, socialization, and emotional well-being
• Document assessment findings, treatment plans, and interventions accurately and timely in accordance with regulatory standards and organizational policies
• Collaborate effectively with facility staff, including nurses, physicians, and administrators, to coordinate long term care plans, address resident needs, and optimize treatment outcomes

Requirements :

• Licensed Independent Clinical Social Worker in the state in which you applied (OH, PA, WV, VA, IN)
• Past counseling experience - Long Term Care experience preferred
• Create a climate of patient-centered care to include confidentiality, empathy, respect, and collaboration
• Develop individualized Treatment Plans based on information collected during initial screenings / evaluations and the ongoing assessment of needs, strengths, preferences, and goals
• A strong work ethic, attention to detail, a willingness to learn, and excellent empathy
• Familiarity with the geriatric population and long-care settings is a plus
• Candidates must have excellent skills in diagnostic assessment and counseling and desire to work in a fast-paced team environment
