About The Position

RN-Case Manager-Hospice (Mountain View) - $60 to $65 per hour in Sunnyvale, CA

Registered Nurse (RN) - Case Manager - Hospice / Quick Hire

Location : Sunnyvale, CA

Contract Duration : 13 Weeks Contractual + Extensions

Shift : Days : 8 : 30am-5pm (Schedule can be flexible based on patient's availability.) Weekends : One weekend per month.

Pay Range : $60.00 to $65.00 / hr on W2

POSITION SUMMARY :

The Hospice Nurse Case Manager admits patients to service, assesses the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs.

They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment.

This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed.

Territory Details :

Atherton, Menlo Park, Palo Alto, Mountain View, Sunnyvale, Santa Clara, San Jose, Saratoga, Los Gatos, Campbell, Morgan Hill, Gilroy and neighboring cities as needed.

1 Year of Hospice Nursing Experience Required

Experience

• 1 years’ nursing experience
• 1 year of Case Manager experience

Education Requirements :

• Two-year (ADN) degree in nursing, a four-year degree (BSN) in nursing, or a Diploma Nursing degree.
• BSN preferred.

Certification / License / Registry Requirements :

• Current State of CA RN license or the ability to establish licensure according to CA law prior to start date.
• Current BLS certification for Healthcare Provider from the American Heart Association.

The employee is responsible for providing specialized care for the facility in need. The employee works in collaboration with all clinic members to ensure every patient receives the safest care with the highest quality optimal outcomes.

This is a contracted travel position with the possibility of being extended.
