What You Can Expect :

The Nurse Navigator (NN) helps patients navigate the healthcare system, addressing barriers such as housing, jobs, and transportation.

The NN conducts pre-visit planning (PVP) appointments before a patient's first visit with the physician to review medications, allergies, health history, care team members, and necessary resources or assistive devices.

Key responsibilities include :

• Escalating patient needs to the care team when significant resources are required.
• Assisting with resources, FMLA / disability paperwork, coordinating meetings, and tracking referrals.
• Collaborating with providers and care teams to ensure accessible care.
• Supporting staff nurses as needed and reviewing referrals for triage.

Scheudle : Monday-Friday 8 : 00am-5 : 00pm

Qualifications :

• Education : Nursing Degree
• Experience : 2 years's of RN experience
• Licenses / Certifications : Current RN License in the State of Practice AND Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
