Sunbelt is currently seeking an HomeCare HomeBase (HCHB) Registered Nurse for a Home Health contract position in O'Fallen, IL.

Responsibilities :
- Utilize expert knowledge of HomeCare HomeBase, Oasis andOutlook, including Start of Cares / Admissions, Re-certifications, Resumptions of Care, Transfers, and Discharges.
- Complete accurate and detailed HomeCare HomeBase andOasis documentation within a 24-48-hour turnaround time.
- Case manage a workload of at least 5-7 patients per day
- Travel to patients homes in the O'Fallen, ILarea.
Experience :
- At least two years of recent Home Health experience.
- Expert knowledge and field experience of HomeCare HomeBase required.
- Start of Care / Admissions experience required.
- Experience in case managing at least 5-7 patients per day
Education :
- Associates Degree in Nursing or Bachelors Degree in Nursing.
- Illinois Registered Nurse License or compact state license.
- American Heart Association Basic Life Support certification.
Employment Type : Full-time, Contract, 13 weeks
Work Hours : 8 : 00 AM - 5 : 00 PM, 40 hours per week, with some on-call and weekends required.
