General Statement of Duties
The Team Lead works along-side the Residential Manager to develop, implement, and coordinate supports that address the needs and interests of consumers and assist them in safely reaching their personal goals for skill development and community integration.
Responsibilities :
Essential Job Functions
Provides direct care when needed to ensure that consumers get to medical appointments, programs and activities and receive daily care as intended.
Schedules medical appointments for consumers and accompanies them; ensures emergency care is received as needed.
Conducts regular consumer meetings.
Provide direct care and support to individuals with disabilities or special needs
Assist with daily living activities such as bathing, dressing, and grooming
Implement behavior management strategies and provide emotional support
Assist with meal planning, preparation, and feeding
Support individuals in participating in recreational activities and community outings
Document individual progress and report any concerns or incidents to the supervisor
Routinely meets with supervisor and participates in management meetings and activities.
Qualifications :
Required Skills & Qualifications :
Minimum High School Diploma / GED
Experience working with individuals with autism, hospice care, or senior care is a plus
1-year experience working with developmentally disabled individuals
Computer literacy using Microsoft Word; familiarity with an electronic health record system preferred
Knowledge of behavior management techniques and strategies
Ability to lift and transfer individuals as needed
Strong communication skills to effectively interact with individuals, families, and other team members
Understanding of resident rights and ability to advocate for individual needs
Must be 21 years of age or older
Two (2) years experience working with persons with physical and / or developmental disabilities; 1 year experience as supervisor
Ability to understand and implement individualized supports

USD $20.80 - USD $21.80 / Hr.
