$231,000 to $280,000 per year
About The Position
An outstanding full-time opportunity for a Board-Certified Family Medicine or Internal Medicine Physician with Geriatric Medicine Fellowship training or CAQ in Geriatrics to serve as Medical Director of a Program of All-Inclusive Care for the Elderly (PACE) in Mishawaka, Indiana.
This leadership role oversees the medical direction of the program, ensures high-quality care delivery, supervises clinical providers, and drives quality outcomes for a frail elderly population.
π©Ί Key Responsibilities
Provide medical direction & oversight of participant care.
Supervise Primary Care Providers (PCPs) & APPs.
Develop & maintain medical policies, procedures, and protocols.
Lead Quality Assurance & Performance Improvement (QAPI) initiatives.
Conduct physicals, reassessments, and develop treatment plans.
Participate in interdisciplinary team meetings & family conferences.
Educate patients and families on chronic conditions & care planning.
Collaborate across divisions and with community providers.
Ensure accurate clinical documentation & compliance.
Participate in provider on-call rotation as required.

Job Requirements
Board Certified in Family Medicine or Internal Medicine β Required.
Board Certified in Geriatrics (Fellowship or CAQ) β Highly Preferred.
Active Indiana medical license (or ability to obtain).
CDS & DEA certification (by start date) β Required.
BLS certification β Required.
Minimum 1 year experience with frail / elderly population β Required.
Prior supervisory experience β Required.
No visa sponsorship (J1 / H1B not supported).
