You’re a licensed LVN in Texas who knows home health inside and out. You’re the kind of person who can take a referral, assess clinical appropriateness, and coordinate care like a pro - all while keeping patients, families, and clinicians in the loop. You’re calm under pressure, sharp with documentation, and you know how to get things done without drama.

    • What You Have to Be Okay With :

  • Working independently on weekends (Saturday & Sunday, typically 8 AM-5 PM).
  • Handling intake, documentation, and coordination with minimal handholding.
  • Communicating with account executives, hospitals, and field staff to ensure smooth admissions.
  • Using AXXESS EMR systems and juggling multiple priorities in real time.

    • Critical Qualifications :

  • Current Texas LVN license (required).
  • At least 1 year of recent experience in home health, intake, or case management (preferred).
  • Solid understanding of Medicare, Medicaid, and private insurance guidelines.
  • Strong communication, organizational, and customer service skills.
  • Proficiency with AXXESS EMR systems and Microsoft Office.
