Per Diem / PRN Physical Therapist -Hawthorne, CA

Hourly Rate : $55 - $65

Type of Work : PRN or Per Diem / PRN

Set up : SNF or Skilled Nursing Rehabilitation

Requirement :

1. Graduate of an American Physical Therapy Associations accredited Physical Therapy degree program

2. Valid and active license to practice Physical Therapy in the state of California.

3. Current CPR certification.

4. Previous experience working as a Physical Therapist, preferably in a travel or diverse clinical setting.

5. Authorization to work in the United States.

6. Excellent communication and interpersonal skills, with the ability to effectively interact with patients, families, and interdisciplinary team members.

7. Demonstrated ability to work independently and collaboratively in a fast-paced environment.

Job Summary :

The Physical Therapist assesses, plans, implements prescribed physical therapy treatments and supervises individual patient care.

Provides quality services autonomously in a Hospital setting. Assists patients to prevent disability and aims toward the promotion of maximal patient / caregiver participation.

Is responsible for recovery programs following disease, injury, surgery or loss of a body part. Provides care according to Physical Therapy, Medical Center policies and procedures and the application of treatment techniques in accordance with discipline specific designated practice act.

Provides in-patients services under physician's orders.

Job responsibilities :

Conduct thorough assessments to evaluate patients' physical and functional status.

Develop individualized treatment plans based on assessment findings and patients' goals.

Implement evidence-based therapeutic interventions to promote mobility, strength, and function.

Monitor patients' progress and modify treatment plans as necessary to achieve desired outcomes.

Educate patients and their families on exercises, techniques, and strategies for self-management and injury prevention.

Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans.

Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and ensure continuity of services.
