A home care organization in Washington is seeking a licensed Nurse Practitioner (NP) and / or Physician Assistant (PA) to join their team to provide support in the Puyallup area.
About the Opportunity :
- Schedule : Monday to Friday
- Hours : 8am to 5pm
- Patient Population : Geriatrics
- Patient Volume : 6 to 8 per day
- EMR : ECW
Responsibilities :
The Primary Care Provider (NP / PA) will :
- Provide compassionate, holistic patient care, addressing diverse needs and collaborating with multidisciplinary teams
- Deliver exceptional clinical care in-person and virtually, with in-person settings including private homes, congregate living facilities, and / or other home settings
- Integrate practices and processes to achieve company goals
- Maintain accurate medical records and participate in initiatives for patient engagement
- Collaborate with external providers and partners
- Perform other duties, as needed
Qualifications :
- Graduation from an accredited Health Professionals school
- NYS Nurse Practitioner (NP) and / or Physician Assistant (PA) license
- Successful completion of national Board Certification
- Active DEA licensure and state-specific Controlled Substance registration, as applicable.
- BLS certification
- Clinical experience caring for aging Adults and Adults with complex care needs
